Ombudsman calls sample ABN "accurate"-information provided by our friends at AMEPA.

The http://www.dmecompetitivebid.com/ website states: if a non-contract supplier that furnishes a competitive bid item in a CBA obtains a signed Advance Beneficiary Notice (ABN) indicating that the beneficiary was informed in writing prior to receiving the item or service that there would be no coverage due to the supplier's non-contract status, and the beneficiary agrees to be liable for all costs, the non-contract supplier may charge the beneficiary for the item or service.

Without an official sample statement of "Reason that Medicare May not pay for the item", AMEPA sent the attached ABN to the Miami Ombudsman for approval.

The ombudsman called the statement "simple and accurate". She warned however: "please understand that I cannot advise you on how to complete forms or determine what is or isn't acceptable. Those questions should be directed to the DME MAC that processes your claims."
