The http://www.dmecompetitivebid.com/ website states: if a non-contract supplier that furnishes a competitive bid item in a CBA obtains a signed Advance Beneficiary Notice (ABN) indicating that the beneficiary was informed in writing prior to receiving the item or service that there would be no coverage due to the supplier's non-contract status, and the beneficiary agrees to be liable for all costs, the non-contract supplier may charge the beneficiary for the item or service.
Without an official sample statement of "Reason that Medicare May not pay for the item", AMEPA sent the attached ABN to the Miami Ombudsman for approval.
The ombudsman called the statement "simple and accurate". She warned however: "please understand that I cannot advise you on how to complete forms or determine what is or isn't acceptable. Those questions should be directed to the DME MAC that processes your claims."

To Order Home Medical Equipment for Your Patient
Competitive Bidding has made it more difficult to order home medical equipment for your Medicare patient, we have created a website that can help you locate a provider in your patient’s area for the product you are needing to order. If you go to www.medicaresuppliersnetwork.com, there are numerous ways to locate a provider.
1. Click on the Locate a Supplier area, type in the patient’s zip code, and a list will appear in order of the provider who is closest to the patient
2. Go to the CBA area on the home page, click on the correct CBA and the list of providers will appear in alphabetical order with products that they can provide
3. BEST OPTION-Click on the Equipment Request Form and complete a few pieces of information and a fax ready document will be provided for you that will pre-populate with the company closest to the patient that you are able to send directly to the contracted supplier to start the ordering process.
